Appointments are generally available Monday through Friday from 9:00 AM to 5:00 PM. Sessions are 50 minutes. To make an appointment, please call our office at (480) 607-7852.
Because sessions are part of your therapist’s schedule, we require at least 24 hours notice to cancel your appointment. Please call 24 hours in advance of your scheduled appointment time. If you cancel your appointment less than 24 hours in advance of your scheduled appointment time, you will be charged for the appointment.
Elizabeth Marietti is contracted with several insurance providers, however many people may be able to receive insurance reimbursement via “out of network” benefits. We encourage you to contact your insurance company to identify your out-patient mental health out-of-network benefits. If you request it, we will provide a statement suitable for submission to your insurance company for filing a claim. If your insurance company requests a diagnosis, it will require a code. Your diagnosis may not qualify for insurance coverage benefits.
Payment is due at the time of service for each appointment. We accept credit card, check and cash payments.
We adhere to state and federal laws along with industry-standard ethical guidelines governing privacy. We can only acknowledge your existence with your expressed, written consent. If anyone requests information regarding you, we will discuss this with you. For us to talk with interested parities, you would need to sign appropriate forms giving us your consent to talk with the identified parties.
